(Spoiler alert: probably ALL the time.) So, let’s grab this keto milkshake recipe – it’s time for dessert!
This keto shake recipe is cold, frothy, and sooooo creamy thanks to a rich blend of heavy cream and coconut milk, along with some vanilla and powdered monk fruit allulose blend. It’s everything you want in a low carb shake, and it’s perfect for sharing. (You can also try other types of keto friendly milk here.)

You are less than 5 minutes and 2 steps away from making the most amazing low carb milkshake ever!
- Toss all ingredients (except ice) into a blender. Blend until smooth.
- Add ice cubes and pulse.
TIP: Don’t over blend the ice because it will make the shake watered down.
I like to add a bit of whipped cream and berries on top, but this is optional. Sit back and sip on your yummy keto shake!
TIP: If you want the low carb milkshake super thick, you can freeze it for about 30 minutes and then blend again.
Sugar-Free Milkshake Variations
One thing that makes this keto milkshake recipe stand out is that it’s super versatile and you can do a lot with it.
Try out some of these ideas to give you something new to taste:
- Strawberries – Either use a strawberry or two as a garnish or toss a few in the keto milkshake. So good!
- Peanut Butter – Add 1-2 tablespoons of peanut butter to this keto shake recipe. It’s perfect if you are looking to add a little more protein and love peanut butter flavor.
- Cinnamon – Sprinkle a little bit of cinnamon into the shake. This is especially good during the fall and winter months.
- Cocoa Powder – Put 1 or 2 tablespoons of cocoa powder on those days when you are craving a little bit of chocolate.
- Instant Coffee – If you are drinking this shake in the morning or are just craving coffee, put up to 2 teaspoons of instant coffee in the keto vanilla shake. It will taste like a drink from a fancy coffee shop. It goes well when you pair it with cocoa too.
- Avocado – Adding some avocado makes the low carb milkshake even creamier. Plus, it adds a lovely flavor to the best keto shake.
- Raspberries – You can’t go wrong adding a few raspberries to the ketogenic shake. Plus, it makes it a lovely pink color that everyone goes crazy for.

0I followed the recipe exactly but it wasn’t thick so I added 1/4 top of xanthan gum and a drop of liquid special, and some Torani sugarfree vanilla syrup. Thank you for the recipe.

0All I have are 15 ounce cans of cream of coconut. How much cream should I use in place of the 13.5 can of coconut milk?

0Hi Erica, Cream of coconut has a lot of sugar, so I don’t recommend it for keto recipes.

0Delicious. I used just Allulose and since I didn’t have coconut milk I used the little bit of macadamia milk and unsweetened vanilla almond milk mixed together. The shake was yum!

0I have made this twice and each time everything has curdled and there’s a huge lump at the bottom of my blender. The liquid tastes good but I could only finish the smoothie with a spoon but it looks so revolting that I can’t even attempt it

0I have been craving a milkshake for EVER! I didn’t think about making one, till I came across the picture of this on the website. I made it so fast and it was delicious! I put some ground up sugar free choco chips to add a little crunch. Next time I will chop up some fresh strawberries. I was SO full afterward, cuz I had just eaten the broccoli cheddar soup, which was AMAZING! Headed over to leave a review for that as well.

0Yummm! I don’t love coconut so I wasn’t sure how I was gonna like it. It definitely tastes more like a coconut milkshake than a vanilla one but it’s DELICIOUS! Exactly what I needed to curb my ice cream craving. Oh also, I didn’t have enough heavy cream so I used half a cup of unsweetened vanilla almond milk and it came out amazing!

0I am confused how you came to your calorie count at 379 as 1 cup of heavy cream alone is 800 calories and 1 cup of canned of unsweetened coconut milk is 510 those alone are over 750 calories a serving what am I missing?

0Has anyone tried freezing the coconut water in ice cube trays and using it instead of plain ice cubes? Is throwing the coconut water out a matter of “too watery” or flavor? I dislike waste & would rather have more coconut flavor instead of diluting the flavor with plain water ice cubes, if it works. Thanks, Everyone, for your comments & suggestions! Having a sweet, creamy treat helps me feel like I’m living big instead of “giving up” foods to stay healthy.

0Hi Denieses, I have not tried that but it should work fine. Your milkshake will not be quite as thick and creamy, but the recipe will still work.

0This was delicious! Quick question…the recipe calls for 1/4 besti, which is 12tsp at 3g carbs each= 36 g carbs, yet the recipe is 4 servings and only 4g carbs. Am I missing something? I’m not sure if the allulose negates the carbs? I ended up using 2 tbs monk fruit granular sweetener at 6g carbs, because that is all I had, it was actually a little on the sweet side, but delicious. Just curious as I’m really trying to stick to 20g carbs a day.

0Hi Theresa, Allulose is 0 net carbs. You subtract the allulose from the carb count to get 0 net. I hope this helps!

0Was amazing! Was looking for a low carb snack for between meals. I struck gold with this one. I don’t have ice trays so, I just mixed the drink then put that in the freezer. When it had thickened I put it back in the blender and it worked out great. Sometimes I will add raspberries or other low carb fruit for a changeup. Thanks again this is a great snack/meal substitute.

0I refrigerated my can of coconut milk and added the entire thing to the blender, and reduce the heavy whipping cream to 3/4 cup. The only sweetener I had that was keto approved is erythritol, which I used the 1/4 cup as directed. I used crushed ice to reduce the blending time. **Busy tip** After thoroughly enjoying the one cup serving, I froze three portions of 1/2 cup, and left the remaining 1 1/2 cups covered in the fridge. When I make my next shake, I will add 1/2 cup of the liquid, with one portion of the frozen to the blender to get that milkshake texture (to equal 1 cup serving)

0This is excellent. We have made it several times now. We loved the creaminess. I only added about 3/4 to 1 cup of crushed ice. We made it without adding anything and was great, also tried added a few raspberries once and a little cocoa powder and that was very good also. Can’t say enough good things about the recipe!

0Awesome recipe! I have a couple of tips. First off if you are going to put it in a jar in the freezer make sure it’s “freezer-safe” all the newer jars are as long as they don’t have the “shoulders” even leaving space once they start to come back to refrigerated or even room temp they will shatter. The jars that are straight up and down and say specifically freezer safe. Just wanted to safe someone that enormous task of either cleaning their freezer because the darn thing shattered in there or in the fridge or on the counter, makes a heck of a mess and you whatever was stored MUST be tossed because it could have unseen slivers of glass in it. To avoid this altogether and what I like to do especially for drink recipes is take the favorite glass that I like to have my drinks in and insert a medium size ziploc bag then pour the pre-made drink above…minus the ice…into the ziploc bag and you have a perfectly portioned drink that if you then put it in the freezer after carefully sealing, then when you take it out of the freezer it thaws really quickly and before completely thawing you can toss it in the blender and whip it up and instant ice cold drink that isn’t watered down with ice. Of course you can still add the ice to keep it that extra chilled for longer, also another tip so you don’t have a lot of these bags hanging out in your freezer I like to make batches of my favourite drinks. I will make your recipe above I will double or triple and then I’ll portion out into the individual bags and label and date. You then have a couple of choices you can push the drink down to the bottom of the bag and removing all the air seal it which makes it super easy to unbag straight into the blender while still a frozen chunk or you can flatten the bags removing as much air as possible . I really depends how you like to organize you freezer etc. I like to take my flattened bags and put them altogether into a larger ziploc bag and that way I can easily find my drink mix bags. This allows me to know when I’m running low I can make another batch and not lose any stray bags at the back or bottom of the freezer. I hope my tips are helpful. I would never have thought of them unless someone told me to try them this way …thanks hubby! So smart!

0The recipe calls for powdered erythritol which is said to be 70% as sweet as sugar. The link for that in the recipe goes to swerve which is said to have an additive making it as sweet as sugar. The conversion calculator for 6tbsp erythritol says to use 1/2 cup swerve confectioners (8tbsp). Are the sweetness percentages different for the powdered sweeteners?

0Hi Nick, Sorry about the confusion. This keto milkshake recipe has been updated with the sweetener I recommend now and should avoid any confusion. However, if you want to use powdered erythritol or powdered Swerve instead of Besti, you can do that by converting between the powdered sweeteners in the low carb sweetener conversion calculator here.

0Mmmm add some cocoa powder and you’ve got a chocolate milkshake. I make this but skip coconut cream cause its yucky to me and use 1/2 cup of fairlife brand mike which is lactose free and has half the carbs of regular milk.

0Hi Kelly. I think vanilla protein powder would work just fine. You may have to adjust or eliminate the vanilla extract though. The link on the recipe card will take you to the 13.5 ounce can of coconut milk that I prefer to use.

0Hello! Thank you so much for your amazing recipes! I tried this and it tasted just like ice cream. It will definitely help with my sweet tooth.
Now, I purchased the same coconut milk you linked here and the directions state to use the cream and discard the water. I put the can in the freezer for about 15 minutes, and when I opened it, the cream layer was very thin with a LOT of milk left (no water left, like the recipe states – it was all milk), so I decided to add a little more coconut milk (but recipes says to use a whole can?). It felt like a waste to discard the rest of the milk, so I put the rest in the fridge for future use. Am I doing something wrong? Based on the directions, I expected to find a lot of cream on top and water under it, but instead, I found a thin layer of creamy with lots of creamy white milk under it. Did I not cool it long enough? Is cooling it what causes the water and cream to separate? Thanks again!

0Hi Denise, Yes, cooling separates the cream and coconut water. Typically I put it in the fridge overnight, so most likely 15 minutes in the freezer was not sufficient to separate it. I’m glad you liked the taste!

0Hi Pam, The recipe makes 4 cups, so would not fill 4 mason jars. I liked them for serving. 🙂 However, I did drink mine out of the jar afterward and could barely finish a cup, let alone more. They are VERY filling, so it would be pretty hard to drink a whole mason jar anyway. Maybe share one. 😉
