I’ve made a lot of smoothies over the years, but most fruity ones don’t work on a keto way of eating because they’re loaded with sugar. I created this keto smoothie to fit my low carb lifestyle at the time, and I still make it often today. I think I always will, no matter how I’m eating. Here’s why:
- Sweet and fruity – Strawberries are one of only a few keto fruits, and this keto smoothie recipe lets them shine. It tastes like a classic strawberry smoothie.
- Thick and creamy texture – My (super simple) secret ingredient makes this smoothie rich, smooth, and thick… it’s avocado! And I promise, you can’t even taste it.
- Very keto friendly – At less than 6 grams net carbs, only 102 calories, and almost 5 grams of fiber per serving, this low carb smoothie is incredibly easy to fit into your macros.
- 4 clean ingredients – I always feel good about sipping this. It’s also dairy-free, gluten-free, and sugar-free. And it only takes a couple minutes to make.
This is probably my favorite of all the keto smoothies I’ve made — and that would be many. Make it with me for a satisfying breakfast or snack!

Ingredients & Substitutions
Here I explain the best ingredients for my keto smoothie recipe, what each one does, and substitution options. For measurements, see the recipe card.
Basic Keto Smoothie Ingredients:
- Fruit – I use frozen strawberries to to make the smoothie cold and thick without needing ice cubes. They also happen to be one of the lowest carb fruits on my keto food list. Feel free to use blueberries, raspberries, or blackberries, but you’ll probably need more sweetener. I don’t recommend other fruits, as they’ll be too high in sugar for keto.
- Besti – I use Besti Powdered Monk Fruit Allulose Blend because it dissolves easily and never gets gritty… plus it tastes just like sugar. My Wholesome Yum Keto Simple Syrup works well too, if you want a liquid option. Most other sweeteners (including other brands of monk fruit and stevia) contain erythritol as a filler, which settles at the bottom and can crystallize.
- Avocado – You won’t taste it at all, but it’s my trick for making the smoothie extra thick, creamy, and packed with healthy fats!
- Keto Friendly Milk – Regular milk is too high in carbs, so I use unsweetened almond milk to thin out my keto smoothie. You can also use coconut milk, hemp milk, or even mix heavy cream with water.

Optional Add-Ins:
You can totally keep this keto smoothie recipe simple, but if you want to boost the nutrition or switch up the flavor, here are a few add-ins I like:
- Yogurt – Creates a creamier texture. Be sure your yogurt is unsweetened; I recommend using full-fat Greek yogurt or this keto yogurt recipe.
- Protein – A scoop of unsweetened collagen, whey protein powder, or even a spoonful of nut butter (peanut butter or almond butter) can make this low carb smoothie more satisfying. If you add nut butter or nuts, you might need a splash more milk to thin it out. You might also like my protein smoothie recipe if you want a bigger protein boost.
- Vanilla Extract – If you’re not using vanilla almond milk, a splash of extract adds subtle sweetness and depth.
- Chocolate – A few tablespoons of cocoa powder will cover up the strawberry flavor and makes this creamy smoothie taste like a chocolaty treat.
- Veggies – Try a handful of spinach, kale, or even sliced cucumber. I add a little at a time and taste in between, to ensure I don’t start to taste the greens.
- MCT Oil – This oil helps support ketosis and keeps you full longer (source). I only add up to a tablespoon per serving, otherwise it can upset my stomach.

My Recipe Tips
- To keep your smoothie cold for longer, freeze your glass for 10 minutes. This is especially refreshing on a hot day.
- Don’t have frozen berries? When I don’t want to wait for them to freeze, I just toss in a few ice cubes with the fresh berries to get that frosty texture.
- Use a high-power blender if you can. If yours is less powerful, you might need more milk and won’t be able to get quite as thick of a smoothie. You can also use fresh strawberries if your blender is too weak to handle frozen.
- Adjust Besti to your taste at the end. The amount you need can vary depending on how sweet your berries are and how sweet you want your keto smoothie.
- The color of this smoothie depends your berries and avocado. This sounds obvious, but I’ve had some people ask me why their keto smoothie isn’t as bright pink as mine. I get the color shown here when my berries are super red and my avocado is more yellow-ish. Paler red strawberries or a greener avocado makes the smoothie less bright, but still just as delicious!
- Want it a little fancy? Garnish your drink with a fresh strawberry, a swirl of sugar-free whipped cream, and a colorful straw (I like these fun ones that are also biodegradable).

- Storage: This keto smoothie looks best fresh, since it gets thinner when the berries thaw and the avocado starts to make it yellow-ish with time. However, you can store it for up to 1-2 days in the fridge if you need to. Blend again before enjoying.

0Hi Debbie, You can change the number of servings to “1” on the recipe card and it will show you the amounts of everything. For ingredient substitutions, you’d need to enter the ingredients you’re using into an online calculator, or you can count it by checking the carb counts in my low carb food list. 0Hi Beth, You can store it for several days in the fridge. The color might change a little because of the avocado. If it separates, just whir it in the blender to refresh.

0I am really new to low carb keto and am determined to work it for several reasons, the first of which started out to be loosing weight, but also eating fresh. Which brings me to my question, is there a problem with my using fresh strawberries or blueberries instead of frozen? Is there a reason for using frozen? Simply curious. Looking forward to trying this recipe as the one I was making had enough carbs for a week, lol! Glad I finally sat down and counted them up! No wonder I wasn’t losing weight. Thanks.

0Hi Deborah, The main reason that the berries are frozen is so that the smoothie is ice cold. If you don’t care about that, you can use fresh. Let me know how you like it!

0Yes, you can, though I wouldn’t consider it low carb at that point. It’s up to you to decide what you’re comfortable with. A medium banana has 27g total carbs (24g net carbs), so adding it would add 5.4g total carbs or 4.8g net carbs per serving, bringing the total to 17.4g total carbs or 11.8g net carbs per serving.

0Hi Jodi, There isn’t a good substitute in this recipe, but you can’t really taste the avocado in it. You could try increasing the almond milk and reducing the avocado amount, or replace some of the avocado with some coconut cream.
